VAN DER ZWAN, Anneke nee Mazereeuw
Born January 6, 1938 in Enkhuizen, Netherlands, passed away September 4, 2009. The family of the late Mrs. Ann Van Der Zwan would like to thank all those who so lovingly cared for her and supported her family during her many years of illness. In particular, our heartfelt gratitude goes to the nurses, care aids and dietician staff of the Maple Hill Extended Care Facility for going above and beyond the call of duty to ensure that Anneke received the best care possible. We are truly grateful for all you have done.
